Key Findings

  • 1

Over 80% of India's cataract surgical backlog sits in Tier 2, Tier 3, and rural markets — largely unserved by organised chains

  • 2

Paediatric eye care and low vision rehabilitation are the most underprovided sub-specialties nationally

  • 3

Rising screen time is driving a surge in myopia among urban under-18s — a largely unaddressed outpatient opportunity

  • 4

Diagnostic-led eye care models are outperforming traditional OPD-first approaches in new market entry

  • 5

Insurance penetration for eye procedures remains below 12% — affordability is a solvable barrier, not a structural one

The Scale of India's Eye Care Gap

India accounts for approximately 20% of the world's blind population — an estimated 12 million people — despite representing only 18% of global population. The vast majority of this blindness is preventable or treatable. Cataract alone accounts for 66% of blindness in India, yet the surgical backlog continues to grow because the infrastructure to address it is concentrated in metros and large Tier 1 cities. The organised eye care sector — chains like Dr. Agarwals, Centre for Sight, and Sankara Nethralaya — has made significant progress in the top 20 cities, but coverage drops sharply beyond that.

Research Methodology

This report draws on primary research conducted across 24 cities in India, including 6 metros, 10 Tier 2 cities, and 8 Tier 3 markets. Research included structured interviews with 62 ophthalmologists and optometrists, patient surveys across 180 respondents, facility audits at 18 eye care centres, and analysis of NPCB data and published epidemiological studies. Fieldwork was conducted between February and May 2026.

Frequently Asked Questions

How big is the eye care market in India?

India's organised eye care market was estimated at approximately ₹12,000 crore in 2025, growing at 14–16% CAGR. The total addressable market — including unorganised providers and unmet demand — is significantly larger. The cataract surgery segment alone represents a multi-thousand crore opportunity if the surgical backlog in Tier 2 and Tier 3 markets is addressed.

Which cities have the biggest eye care opportunity in India?

Beyond the top 8 metros, our research identifies Jaipur, Lucknow, Bhopal, Nagpur, Coimbatore, Visakhapatnam, Indore, and Patna as the highest-opportunity markets for organised eye care entry, based on population size, current specialist density, and insurance penetration.

What are the main barriers to eye care access in India?

The primary barriers are geographic — most organised eye care infrastructure is concentrated in metros. Secondary barriers include low awareness of treatable conditions, limited insurance coverage for eye procedures (below 12% penetration), and a shortage of trained ophthalmologists outside major cities.
