The findings provided a market entry and positioning framework for a MedTech company evaluating the Indian robotic surgery segment.

The Endoscopy Market: Volume, Growth & Procedure Mix

The endoscopy market in India's organized hospital sector is growing at 18–22% CAGR, driven by three converging forces: rising GI cancer screening awareness, the rapid expansion of bariatric surgery programs, and the shift toward minimally invasive approaches in gynecology and urology. Our procedure volume mapping across 8 cities found that upper GI endoscopy and colonoscopy account for 55% of total endoscopy volumes, with therapeutic endoscopy (ERCP, EUS, endoscopic submucosal dissection) growing fastest. The concentration of high-volume endoscopy in 15–20 tertiary hospitals per city creates a clear target universe for MedTech companies — but also a highly competitive, relationship-driven procurement environment.

Surgeon Readiness and the Training Bottleneck

Despite high awareness of robotic surgery platforms (Da Vinci, CMR Surgical, SS Innovations), only 12% of surgeons in our survey sample had hands-on robotic experience. The primary barrier is not skepticism — it is access to structured training. Surgeons consistently cited the absence of certified training centres in India, the cost of international proctorship programs, and the lack of simulation labs as the key reasons robotic skills remain concentrated in a small cohort of early adopters. This finding has direct implications for market development strategy: companies that invest in training infrastructure and domestic proctorship networks will accelerate adoption more effectively than those focused solely on capital sales.

The Capital Cost Barrier and the Volume Threshold

Hospital administrators are not opposed to robotic surgery — they are opposed to capital expenditure without a clear volume-based ROI model. Our research found that 68% of administrators cite the ₹12–18 crore acquisition cost as prohibitive in the absence of guaranteed procedure volumes. However, hospitals with existing laparoscopic volumes above 800 procedures per year show 3x higher robotic adoption intent — because they can model a credible utilization pathway. This suggests that the market entry strategy should prioritize high-volume laparoscopic hospitals and position robotic systems as a volume upgrade rather than a new capability.

Technology Preferences and the Case for Flexible Robotics

Not all robotic platforms are perceived equally. Our conjoint analysis revealed that surgeons express strongest interest in flexible endoscopy robots and single-port laparoscopic systems — perceived as more accessible, less infrastructure-intensive, and more immediately applicable to their current case mix. Full robotic platforms (Da Vinci-style) generate interest primarily among oncosurgeons and urologists in high-volume tertiary centres. This segmentation has clear implications for product positioning and the sequencing of market development efforts across surgical specialties.
